Report suggests Knee Meniscus Surgery may be needless: 

Take Physiotherapy as first priority

It starts with pain in the knee, followed by swelling so intense that a patient may have trouble bending or straightening the leg.

A torn meniscus – the cartilage that cushions the knee joint – is a common knee injury in Canadians aged 45 to 65. But patients may be better off without the most common treatment: surgery.

The benefits of knee meniscus surgery are meagre compared with the risks, according to an analysis published Tuesday in the medical journal BMJ. The review of 18 studies found that surgery-related complications, though rare, include deep vein thrombosis, infection or blockage of the main artery of the lung. Even in cases where surgery helps, the benefit is “markedly smaller than that seen from exercise therapy,” the authors wrote.

For middle-aged Canadians, this popular surgery has become a default solution to painful wear and tear on the meniscus. Over the past seven years, however, study after study has shown that arthroscopic surgery, which involves small incisions and a tiny camera that helps the surgeon trim any ragged cartilage, is no better than a placebo.

Physiotherapists note that surgery does not treat the underlying causes of a damaged meniscus, which may include excess body weight and weak musculature around the knee joint. Expanding medical coverage to include physiotherapy would be a more effective strategy to treat age-related knee pain, they argue.

Non-surgical treatments have potential to provide the same, if not better, outcomes for patients, with less risk, “and certainly with less cost,” said Dr. Michael Hunt, an associate professor of Physical therapy at the University of B.C.

Dr. Moin Khan, a research fellow in orthopedic surgery at McMaster University, agrees. He is the lead author of a 2014 study on arthroscopic surgery published in the Canadian Medical Association Journal. Khan and co-authors reviewed how a total of 805 participants in seven separate studies fared after receiving arthroscopic surgery, no surgery or a sham procedure, in which surgeons made an incision but did not trim any cartilage in the knee.

At the six-month and two-year mark the three patient groups showed no significant differences in their pain symptoms or ability to do everyday activities, such as walking or climbing stairs.

“Patients in non-operative groups did just as well, or better in some cases, as the operative group – and definitely at less risk,” Khan said.

Asked why specialists continue to recommend arthroscopic surgery, Khan explained that studies casting doubt on the standard knee scope are relatively recent. “There is often a lag between research findings and changing widespread clinical practice.”

Knee arthroscopy is day surgery that usually takes an hour or two. The surgery is normally performed with local anesthetic around the knee, or regional anesthetic, which numbs the patient from the waist down. The recovery period is about one to two weeks, followed by three to six weeks of Physiotherapy to help restore the full range of motion and strengthen the muscles around the knee.

Physiotherapy treatment without surgery typically involves weekly sessions with a physical therapist in addition to exercises at home. Depending on the patient, exercises may target the quadriceps, hamstrings, calves, hips and pelvis muscles, with the aim to improve kneecap alignment and range of motion of the joints. Many patients have less knee pain after six to eight weeks, but Physiotherapy treatment may take longer for others, especially those who need to shed excess body weight.

The lack of benefit with arthroscopic surgery may be due to the fact that many patients suffer from osteoarthritis, a degeneration of joint cartilage and the underlying bone, as well as a thinning and weakening of the meniscus. Even if a meniscal tear shows up on an MRI scan, it may not be the source of the patient’s pain, Khan said. “Orthopedic surgeons are often challenged to determine the true cause of patients’ symptoms: the meniscal tear, osteoarthritis or a combination of both.”

Treatments for osteoarthritis are the same as non-surgical interventions for a damaged meniscus. They include weight loss and Physiotherapy, often combined with anti-inflammatory medications and cortisone injections to help reduce pain.

Keyhole surgery may help younger patients in their 20s and 30s with meniscal tears from acute sports injuries, according to a 2013 Finnish study. But older patients with knee pain should try alternative treatments for at least three months before considering surgery, Khan said.

Patients with time-worn knees may benefit from using custom orthotics in their footwear, Hunt said. Often, Physiotherapists can help patients strengthen specific muscles to stabilize the knee joint, or adjust their movement patterns to reduce the load on the knee joint. “Exercise is the cornerstone treatment,” he added.

103-year-old on track after 2 surgeries

At the age of 103, Bharpayi Devi is still going strong despite a hip surgery and replacement, all of which was done in less than six months.

It was late last year when an otherwise active Bharpayi Devi, who is a great grandmother, slipped and injured her left hip.

“We thought she wouldn’t survive the surgery, but she beat all odds and was discharged within four days of the surgery,” said her family members.

However, in April, she fell again and broke her right hip.

“She was rushed to Max Super Speciality Hospital, Shalimar Bagh, and we had to schedule a hip replacement surgery,” said Dr. Rajiv Maheshwari, senior orthopaedic at the hospital. This would be her second surgery in a span of six months.

According to doctors, her hip ball was broken and needed to be replaced in order to make her stand and walk.

The surgery proved to be a challenge for Dr. Maheshwari and his team as Bharpayi Devi’s bones were extremely fragile and severely osteoporotic. Also, because of her age administering anaesthesia posed another challenge.

“The patient pulled through and was discharged within four days of surgery. She also took her first step within a week. After two weeks of physiotherapy, Bharpai Devi is back on her feet and has begun doing little chores around the house,” said the doctor who treated her. 

Explaining the importance of the case, the doctor said it highlighted the numerous problems senior citizens face today.


“Loss of vision, brittle bones, inability to heal fast and lot of other irreversible health issues prove fatal for many. Family support is important, especially after the age of 70-75 years,’’ said Dr. Maheshwari.

In India, Diabetes and other Musculoskeletal disorders have replaced diarrheal diseases and vision problems as leading causes of years lived with disability (YLDs) - a measure of overall disease burden, expressed as the number of years lost due to ill-health.

Between 1990 and 2013, years lived with disability among women with diabetes increased by 109%, whereas from other Musculoskeletal disorders increased by 110% in India



The study was conducted by researchers working on the Global Burden of Disease project and led by the Institute for Health Metrics and Evaluation (IHME) at the University of Washington.

USA:  No matter what's happening with the economy, Physical therapy continues to thrive.

In order to become a physical therapist, candidates must obtain a graduate degree, usually a doctorate in physical therapy. College programs in physical therapy typically involve a combination of coursework and clinical rotations. Additionally, states require individuals to be licensed to legally work as physical therapists.

Doctor of Physical Therapy (DPT) programs train students in the procedures of diagnosis and treatment. Coursework covers topics in anatomy and procedures in physical therapy, such as therapeutic applications and exercise, as well as integration and assessment courses.

Besides graduating from an accredited program, states typically require a passing score on the National Physical Therapy Examination as part of the licensing requirements.

The University of Texas Medical Branch at Galveston offers primary entry-level DPT, post-professional DPT and bridge physical therapy assistant (PTA) to DPT programs. The bridge program, to be introduced this summer, is a special hybrid program with in-person and online classes. UTMB also has a transition DPT program.

"A doctorate of physical therapy is relatively new in this profession," said Steve Fisher, PT, Ph.D., associate professor, department of physical therapy, UTMB. "As time goes on, the pool of physical therapists without a DPT gets smaller because nationwide, that's the requirement."

UTMB physical therapy students do field work at sites around the state and around the country. They have their first clinical experience in their second year, then do three clinical rotations in their final year that are eight or 12 weeks long.

Hiring outlook for Houston-area physical therapists is great, with most UTMB graduates having jobs lined up either before they graduate or soon after, Fisher said.
Jobs for physical therapists were expected to grow 36 percent from 2012-2022, according to the U.S. Bureau of Labor Statistics, which is significantly faster than the national average of 11 percent for all occupations.

"The job market is currently very strong. Most new physical therapists earn salaries in the upper $60,000s or low $70,000s; if you go to work in a more rural area, you likely could make more than that," Fisher said. "A good PT candidate would be someone who wants to work with people but also has analytical skills, is creative, and has good approach to rehabilitation to suit an individual need."

Texas Woman's University's Houston campus offers an entry-level DPT, DPT for practicing licensed physical therapists, and a transitional DPT (tDPT) Ph.D. program for already practicing physical therapists. Entry-level DPT candidates have to do five different clinical rotations from one half-day a week to 12 consecutive weeks. Transitional and Ph.Ds are already licensed, so there are no internship requirements for them.

"By May of their graduation year, one-third of students likely have already committed to a position, and after their licensure exam in July, 90 percent have likely accepted a position and with the rest likely considering how they want to spend their last free summer," said Peggy Gleeson, associate director, School of Physical Therapy at Texas Woman's University's Houston campus. "By August, essentially all new graduates have jobs or multiple job offers. In recent recessions, none of our graduates lost jobs or faced reduced hours. There is excellent job stability."

"The ideal PT candidate is someone who is hard-working both in their studies and profession. They also need to want to continue to learn throughout academic career and after they graduate," said Carolyn Da Silva, coordinator of entry-level study, TWU School of Physical Therapy. "We give students the basic skills they need for their exams and getting a job, but it's up to them to pursue extra education and work to improve the care they give to patients.

"Lifelong learning is important; as we're able to diagnose more conditions and learn more how the body functions. There always is more information out there; it's up to PTs to stay up on literature and changes in health care administration. PTs need to be critical thinkers and have to be able to figure out what a patient's problem is with whatever resources they have available."

Flexibility of profession is what sets physical therapy apart in some ways, Gleeson said. It's fairly easy to change your focus, such as going from outpatient orthopedic to pediatrics or geriatrics, and maintain your standing without losing momentum. There also are opportunities for part-time work if PTs want to get another degree, raise a family, etc. If someone moves to different state or city, it's fairly easy to find a job in another area because PTs are in such demand.
